Care Management Department / Post-Hospital Care
Our Care Management Department can assist you and your family with various post-hospital care issues that might arise during your hospitalization. Our services include:
- Providing supportive counseling to cope with illness and hospitalization.
- Exploring discharge options, such as home care services, homemaker / housekeeping services, “Meals on Wheels”, inpatient rehabilitation, extended care placement, hospice services and durable medical equipment.
- Making referrals or assisting with scheduling recommended follow-up visits with a physician.
- Making referrals for outpatient mental health counseling.
- Managing problems associated with suspected or actual child abuse or neglect, family or domestic violence, and elder abuse or neglect.
- Making referrals to various community resources, such as family service agencies, financial agencies or self-help support groups.
If you determine that you need further Care Management assistance after you are discharged from the hospital, you can contact the hospital’s Care Management Department at 708-824-4668, Monday through Friday, 8:00 a.m. to 4:30 p.m.
