MetroSouth Home Care gives individuals and their care givers the ability to receive medical treatment and care from professional health care providers at home. Our home care services are available 24 hours a day, 7 days a week, with schedules designed to meet the individual needs of our patients and their families.
Home Care Services provides sensitive, individualized care coordinated through an interdisciplinary team that includes:
- Registered Nurses conduct the initial evaluation and physical assessment and then coordinate all of the physician’s prescribed care.
- Home Care Health Aides are certified by the state of Illinois and supervised by a registered nurse.
- Physical Therapists establish a treatment program designed to improve and restore a patient’s mobility and ambulation.
- Occupational Therapists maximize a patient’s ability to perform daily living skills, such as feeding and personal hygiene.
- Speech Pathologists evaluate and rehabilitate language and swallowing skills.
- Medical Social Workers assist with financial and emotional problems that may have developed because of illness or physical impairment.
Additional Services

Referral to MetroSouth Home Care
To make a referral, simply call (708) 371-7777 or TDD# (800)-526-0857 and our experienced Home Care Referral Coordinators will facilitate obtaining the necessary physician orders, information, and verifying eligibility for home care services.
Patients can be referred to MetroSouth Home Care by physicians, case managers, social workers, family members, or at the patient’s own request. An assessment visit can be made to the patient’s home to assist in determining specific home care needs.
