Preparing for Your Visit
To Pre-Register for any scheduled service or procedure call to 708-597-2000 ext. 4855 or ext. 5095. When calling, please provide the following information:
- Full Name
- Date of Birth
- Social Security Number
- Current Residential Address
- Primary Care Physician
- Insurance Carrier, Policy Number, Group Number
- Insurance Address and Phone Number
- Referral Authorization Number (If applicable)
- Please bring the referral with you to your appointment.
Medical Insurance
Please bring your health insurance cards and a photo identification card with you; i.e. Driver’s License or State Identification, and present them to the Registration Representative when you arrive at the hospital. Providing the necessary health insurance information upfront will help make sure your bill is processed and paid in a timely manner.
Patients who have no insurance coverage must contact Patient Financial Services before a scheduled admission date and establish an agreed-upon arrangement for payment of the medical bill. MetroSouth may also identify alternative resources to help the patient meet their financial obligation to the hospital.
Financial Assistance
If you are uninsured or under insured, financial assistance is available. MetroSouth will assist you in determining if you quality for medical coverage through various state-funded programs.
If it turns out you are not eligible for benefits through any these programs, MetroSouth will further review your file for financial assistance program through our internal Charity Care Assistance Program. Simply fill out a Financial Assistance Application and return it our Financial Aid office along with copies of the following documents:
- Drivers License, Citizenship papers or State ID card
- Birth certificate
- Copy of most recent check stub
- Unemployment notification letter
- School records
- Last filed W2 tax documents
- Social Security Card
- Rent/Mortgage receipts
- Bank Account statement (Checking/Savings)
- Trust fund or Family inheritance

Preparing for a Surgical Visit
The doctor’s office will send and order for what pre-testing is required to the pre-operative department at MetroSouth Medical Center. The pre-operative nurse will contact call you and schedule an appointment. Pre-operative testing, interview, and pre-operative teaching will occur at the appointment time. If pre-operative testing is completed elsewhere, the interview will take place over the phone.
